RESPONSIBILITIES<ul><li>Ensures compliance with federal, state, and local regulations, accreditation standards, and hospice policies in all hospice program patient care aspects while optimizing comfort and dignity.&nbsp;</li><li>Provides hospice nursing services, treatments, and preventive procedures, ensuring safe and therapeutically effective services to patients.</li><li>Regularly assesses and reassesses the nursing needs of the hospice patient, ensuring all nursing needs are met, as identified in the patient’s initial assessment, comprehensive assessment, and updated assessments.</li><li>Actively participates in quality improvement reviews and evaluations of nursing services provided, and contributes to IDG meetings as requested.</li><li>Review records for compliance with regulations.</li><li>Ensures the patient is being nourished as appropriate working with the Hospice House team.</li><li>Implements the plan of care and initiates nursing procedures appropriate for the patient’s hospice care and safety.</li><li>Obtains physician’s orders when necessary.</li><li>Observes signs and symptoms and coordinates and communicates reports to the physician and IDG members, any unexpected changes in the patient’s physical or emotional condition.</li><li>Coordinates care with internal and external interdisciplinary team members, including social workers, chaplains, and home health aides.</li><li>Teaches, supervises, and counsels the hospice patient and family members regarding care needs, including pain management.</li><li>Supervises, orients, and trains other nursing service personnel as requested.</li><li>Prepares patient-specific instruction for hospice aides/certified nursing assistants.</li><li>Develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care.</li><li>Provides services requiring substantial and specialized nursing skill; performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor’s orders.</li><li>Evaluates the outcome of services.</li><li>Maintains accurate and complete medical record documentation.</li><li>Supervises licensed practical nurses,&nbsp; hospice aides and/or certified nursing assistants.</li><li>Observes confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.</li><li>Always communicates to the supervisor if unable to meet a patient’s need or perform a&nbsp;</li></ul>procedure.<ul><li>Participates in on-call system and is responsible for providing on-call coverage when</li></ul>unavailable for assigned duties.<ul><li>Maintains skills and knowledge, keeping all required certifications up to date.</li><li>Participates in quality improvement initiatives and continuing education to maintain up-to-date knowledge and skills.</li><li>Organizes work schedule and utilizes time management to be able to attend all required</li></ul>meetings and team conferences.<ul><li>Complies with current infection control policies and protocols.</li><li>Immediately reports any accident, incident, lost articles, or unusual occurrence to the IPU Director/Clinical Coordinator.&nbsp;</li><li>Adheres to the HHHR code of ethical behavior and models our core values of respect, passion, and professionalism at all times</li><li>Performs all other duties as assigned.&nbsp;</li></ul> &nbsp;III.
