Coordinates program services with other service providers and community organizations to promote optimal implementation. Screen and refer clients to other community service providers. Monitoring per program guidelines, educational instruction, materials to promote wellness, safety, and pregnancy and postpartum complications. Monitor for infant complications to include feeding, weight, head circumference, and developmental screening. Develop a plan of care for individuals and families, modify treatment plans as indicated by client’s responses and conditions. Provide referral/linkage when medical and environmental risks are identified to appropriate services. Promote program through community engagement and outreach events.
