This pharmacist will be part of a collaborative pharmacist practice with clinical coverage responsibilities within the Orthopedic, Colorectal, Urology, Bariatric, Plastics, and General Endocrine and Metabolic Surgery service lines. At UK HealthCare the pharmacist makes clinical interventions via patient education, therapeutic drug monitoring, drug information, cost-effective care analysis, direct patient care through interdisciplinary rounds, and interacting with pharmacy and surgery residents and students. The ideal candidate will have graduated from an ACPE-accredited school/college of pharmacy, completed an ASHP-accredited PGY1 residency (or equivalent experience), and must be eligible for licensure in Kentucky. Completion of a PGY2 residency is encouraged.&nbsp; This position provides clinical pharmaceutical expertise for surgical teams through perioperative and post-operative/acute phases of care. The GSH surgery representative is responsible for assisting with transitions of care including resumption of home medications upon admission. The pharmacist will also provide dosage form interchanges and alternative therapy recommendations for surgical patients requiring crushed medications post-operatively. Must work well in a dynamic, complex environment and under deadline pressures.
