Physical Requirements

The physical requirements of this position include: the ability to lift, carry, push, and pull 25-100 pounds on a daily basis; regularly sit at a computer work station for extended periods of time with regular repetitive motions (such as typing); as well as stand, walk and sit for long periods of time. The conditions of work may involve exposure to communicable diseases and injury from intractable patients or sharp instruments.



Shift

Primarily day shift, Monday through Friday, with on-call shift rotations as needed.



Job Summary

The Transplant Nurse Coordinator is responsible coordinating new patient referrals, evaluation, listing, and transplant care management of patients. This process will include multi-organ patients as well as single organ patients. Additionally, this person will collaborate with all team members to provide individualized, multidisciplinary patient care, and participate in quality improvement efforts in alignment with program, department, and hospital strategic plans. The Transplant Nurse Coordinator is a liaison and initial professional point of contact for providers and patients.

This is a remote position located in Louisville, KY.

Excellent nursing care skills; a “self-starter” mentality; organizational skills; a comprehensive knowledge of other disciplines; the ability to prioritize appropriately; and basic to proficient computer/technical skills, including text processing, spreadsheet manipulation, database processes, and Outlook knowledge and abilities.

Preferred Education/Experience

BSN, transplant experience, and CCTC (Certified Clinical Transplant Coordinator) certification preferred.
